Instructions:

1. Click on the link above. Download the file. We will need two (2) completed copies per child both with original signatures.
   
   
2. Division 3 (Advanced - Class III), two (2) copies of child's birth certificate for age verification. If the player is trying out for Class III, please register in advance. After tryouts, the full registration fee will be refunded if the player will be playing for a different club.*
   
   
3. Division 3 (Competitive), two (2) 1"x1" photos, U-10 & up
   
   
4. Fees: $85 per player. $110 after June 1st. Make check out to PVYSC.

*Birth certificate is the official State/County issued certificate, not the one issued by the hospital.
